What’s your current job role, and how long have you been in this position?
I currently serve as vice president of the Continuing Care Department and Care Transitions, overseeing case management and social work services as well as development of the post-acute network for UAB and UAB St. Vincent’s hospitals. I will reach my 10-year anniversary at UAB Medicine in June 2026.
Can you briefly describe the goals and tasks of your Epic Workgroup?
Our workgroup was tasked with developing an approach to care management in the ambulatory setting. We created worklists for identifying high-risk patients and tracking patients with chronic conditions. With those, we will be able to provide care coordination that will continue to improve our readmission indicators.
What’s one accomplishment that you are especially proud of?
I am especially pleased with the collaborative spirit of our team. We included stakeholders from the Department of Medicine campuses at Huntsville and Tuscaloosa, as well as the UAB School of Nursing, and we developed a useful tool that meets the needs of the organization.
Before working in your current role, what was your most unusual or interesting job?
My most interesting job has been here at UAB Medicine. Finding creative solutions to complex discharge planning issues has been one of the most challenging aspects of my role as a Care Transitions leader.
Why are you excited about our Epic Journey?
I believe that Epic will allow for enhanced collaboration among clinical team members. I also sense that Epic will further enable our outreach capabilities, allowing us a finer, risk-stratified approach to longitudinal care coordination.
